KBH Program - Use of KMAP Forms

Effective April 12, 2016, the Kansas Early Periodic Diagnostic and Testing Program (EPSDT) known as KAN Be Healthy has provided updated screening forms.

The forms can be located on the Kansas Medical Assistance Program (KMAP) website at the following link: https://www.kmap-state-ks.us/Public/forms.asp

The Kan Be Healthy – Early and Periodic Screening, Diagnostic, and Treatment Manual can be located on the KMAP website at the following link: https://www.kmap-state-ks.us/Public/providermanuals.asp.

Medicaid KBH providers can continue to use the forms from their EHR as long as those forms follow the Bright Futures guidelines. These summary forms would also satisfy the KBH requirement eliminating the use of the paper form. You may need to contact your EHR company to verify that it follows the Bright Future Guidelines.

KDHE-DHCF is updating the KAN Be Healthy (KBH) Program Manual, the KBH screening forms, website and all related materials for use by KBH providers. KBH follows the Bright Futures periodicity schedule for the Early Periodic Screening Diagnosis and Testing (EPSDT) program.

Bright Futures is CMS-approved and a nationally recognized resource based on the American Academy of Pediatricians (AAP) uniform set of recommendations for health care professionals. Core tenets include delivering health care services that recognize the partnership of the child, family, health care professional and community. In addition to provider resources, Bright Futures offers educational information about child health and wellness to families.
