Money Follows the Person (MFP) was a federal grant that was used by the State to fund and develop a process to transition individuals from approved institutional settings onto the Frail Elderly (FE), Physical Disability (PD), Intellectual/ Developmentally Disabled (I/DD), and Traumatic Brain Injury (TBI) Home and Community Based Services (HCBS) waivers. The MFP program ended on June 30, 2017, and all participants were transitioned off the program by December 31, 2018. As such, this service is no longer available.
Since December 31, 2018, HCBS institutional transitions are no longer being tracked in the Kansas Medical Assistance Program (KMAP) MMIS.
The KMAP provider manuals will be updated appropriately and additional notification will be sent.
