When to Create a New IDD Support Plan
The following information is for providers of IDD Targeted Case Management (TCM) services and is a correction to the information provided during the July 18 and July 20 TCM webinars.
Sunflower has received further clarification from KDADS regarding the Person Centered Service Plan (PCSP) policy and when the new IDD support plan is to be implemented.
TCMs should use the new IDD support plan template and provide a copy to the Sunflower care manager when one or more of the following occurs:
- The member’s existing support plan is about to expire (365 days) and a new plan is being created. This is based upon the effective date of the plan revision is needed (change in support needs for the individual or other change as defined in KAR 30-63-21).
- Required information is missing/absent from a current support plan and needs to be added.
Unless one of the above has occurred, Sunflower care managers will continue to accept existing support plans that are not in the new support plan template for the member’s annual service plan meeting.
