Sunflower’s primary quality goal is to improve members’ health status through a variety of meaningful quality improvement activities.  As we focus on collaborating with providers to generate positive member health outcomes, improve population health and ensure our members are receiving the highest level of quality care, we invite you to participate in our Shoes for Shots Program.

Shoes For Shots Program

Shoes for Shots is a Healthcare Effectiveness Data and Information Set (HEDIS) and Social Determinants of Health related (SDOH) incentive program. It aims to address vaccine hesitancy and improve health outcomes of eligible adolescent Sunflower Health Plan members. The priority vaccine for 2025 is Human Papillomavirus (HPV) vaccine.

How Does It Work?

Sunflower Health Plan makes outreach to members who are eligible for the vaccine/vaccine series, inviting them to participate in the program by scheduling an appointment with a partnering clinic or health department. This may be a stand-alone appointment or during a collaborative event between the health plan and group.  After the member has completed the vaccine (or series of vaccines, if required), prior to the required deadline, they will be provided a new pair of shoes* as a thank you from Sunflower Health Plan.
