Effective with dates of service retroactive to January 1, 2024, diabetes devices, accessories, and supplies will be covered for members ages 20 and under, when managed by a qualified medical professional.
Prior authorization (PA) is required. The services listed below are allowed to be billed by Provider Type/Provider Specialty (PT/PS) 25/250 Durable Medical Equipment (DME)/ Medical Supply Dealer.
Disposable Continuous Glucose Monitors, Accessories, and Supplies:
