You Can Help Our Members Get to Their Appointments with You

Many of our members experience challenges with getting to their healthcare appointments. Sunflower Health Plan works with non-emergency medical transportation (NEMT) to help members overcome those barriers. If you have Sunflower members who have difficulty getting to their appointments with you, encourage them to call us to help them arrange a ride or gas reimbursement.

The company that Sunflower contracts with for NEMT is changing its name to ModivCare. It was formerly known as LogistiCare. The phone number to contact them isn’t changing, nor are the services they provide.

Members must call at least three days before the day of their appointment. They may be able to get a ride sooner if you give them an urgent care appointment. If the appointment is urgent, members must say this when calling to schedule their trip.
