Sunflower Health Plan Partners with Everlywell for At-Home Diabetic Test Kits

Members to receive HbA1c and Kidney Health test kits starting in April. 

We are pleased to share that Sunflower Health Plan has partnered with Everlywell to provide at-home diabetic test kits to members. Everlywell is a leading digital health company that has helped millions of people screen for and manage health conditions.

Sunflower Health Plan members who are due for their HbA1c and/or Kidney health* test will be mailed an at-home test kit in late April/early May. The results will be shared with the patient and you, their healthcare provider, to ensure coordination of care.  You can expect to receive the result via direct message within your EMR or efax.

Sunflower Health Plan members that are excluded from receiving these test kits include:

  • Members who had an HbA1c and/or Kidney health test as of:
    • Medicaid: 4/11/2025
    • Medicare: 4/9/2025
    • Marketplace: 4/7/2025
  • Members who have non-compliant results in the past 9 months (A1c specific)
  • Members with an invalid mailing address
  • Members that term within 90 days of:
    • Medicaid: 4/11/2025
    • Medicare: 4/9/2025
    • Marketplace: 4/13/2025 (members that termed within 30 days)

Additional exclusions for Sunflower Medicaid members include:

  • Members who reside in nursing/long-term care facilities
  • Foster care members

*The kidney health test is a blood and urine test to measure the estimated Glomerular Filtration Rate (eGFR) and urine albumin-to-creatinine ratio (UACR).
