Sunflower IDD Provider Inclusion Grant
Kansas Council on Developmental Disabilities (KCDD), in partnership with Sunflower Health Plan, is excited to announce a new funding opportunity for providers who serve individuals with intellectual and developmental disabilities (IDD).
This grant initiative supports innovative projects that help IDD providers throughout Kansas align with the federal Home and Community-Based Services (HCBS) Final Settings Rule.
The goal is to ensure people with disabilities have full access to community life and receive services in the most integrated and inclusive settings possible.
Eligible applicants must:
- Be Medicaid IDD Day/Residential or Supported Employment Providers
- Serve Sunflower Health Plan members in-person within the Sunflower service area
- Be accepting new Sunflower members
- Be listed in the Sunflower provider directory
