Add a Practitioner to Existing Contract

KanCare Provider Enrollment and Changes: Providers must use the Kansas Modular Medicaid System (KMMS) Provider Enrollment Wizard to update their information for any or all of the Medicaid fee-for-service (FFS) and managed-care organization (MCOs) health plans.

Medicare or Ambetter Add a Practioner to an Existing Contract

1. Download the Facility Roster (Excel) and complete.
2. Download the Disclosure of Ownership form (PDF) and complete.
3. Email completed documents to our network team at rosters@sunflowerhealthplan.com.
