Transition of Care

Welcome to Sunflower Health Plan! We are thrilled to have you as our member.

We know change can be scary. Sunflower wants to make this change easy. Whether you are new to Kansas Medicaid Managed Care or are changing health plans, we will make sure you continue to get the services you need. 

One of the most common obstacles to switching Medicaid plans is maintaining your relationship with the providers that know you best. Our staff will work with you and your doctors to continue any services you are receiving even if the doctor is not in our network.

It is important to let us know right away if you are getting services that you will need to continue so that we can give you the information you need to keep receiving the healthcare you need on our Medicaid Managed Care plans or so that we can help you with transitioning to a new provider.

In some cases you may be able to keep your out-of-network doctor for a period of time. We will help you find a doctor in our network that can provide the services you need.

Our case managers can help get your medical records transferred to your new doctor. Call our Customer Service toll free 1-877-644-4623 (TTY: 711) if you have questions about transitioning your care.

The time after you move from one health plan to another is called the continuity of care period.

We want to help make your move to Sunflower easy! If Sunflower is your new managed care organization (MCO or health plan), please see the FAQs below to help answer your questions.

Services that were in place and authorized by your previous MCO are covered by your new MCO during the continuity of care period. Visit the KanCare website for more information.

If your current provider is not in network, eligible services will be covered during the continuity of care period. 

You can select a new provider by:

Sunflower will send you a welcome packet within 10 calendar days of enrolling with us. This packet includes your member ID card. If you need a card before you get your permanent card, contact Customer Service. Your provider also can verify your coverage through Sunflower or the state.

Show your new member ID card to your pharmacy. The pharmacy will process the claim with Sunflower. If you do not have your new member ID card yet, you can give the pharmacy your Medicaid ID number and tell them Sunflower is your new health plan.

  • Select Member from the dropdown options.
  • Select Medicaid from the dropdown options and click “Submit.”
  • Click “Create New Account.”
  • Follow the directions to register.
